Below are release forms required for some off-site activities.
If a release form is required, you will NOT be allowed to participate without that form. These forms are required by our vendors for admission.
Do NOT send these forms to us! You will either submit them online directly to the vendor or bring them with you to the event.
You will need to fill out any forms required for activities you are attending.

If you can't join us in person, we'll be recording the meetings for you to watch later.

Service Project - Jerome Animal Shelter
Every Jubilation, we make donations to a local organization for our service project. This year, we're collecting donations to the Jerome Animal Shelter!
Their donation wishlist is available on Amazon, though you don't have to purchase from there. Feel free to bring your donation to the event and we'll get everything to the shelter!
Queen of Hearts Fashion Show
Participate in the Queen of Hearts Fashion Show as you show off your best party outfit made out of the best of party and scrapbook supplies! Create your design, name it, and up to three girls can strut down the runway to win the heart of the Queen of Hearts! Examples of party supplies include streamers, party hats, paper plates, paper cups, tissue paper, etc.
Fill out the card at the bottom of the event flyer and had it to the Card Guard at the start of the runway!

SWAPS
What are SWAPS?
SWAPS stands for “Special Whatchamacallits Affectionately Pinned Somewhere.” They are small tokens of friendship that Girl Scouts exchange with one another, and each SWAP uniquely reflects a fellow Girl Scout, their troop, or a memory of a special event. They’re a great way to share stories and get to know Girl Scouts from across the country and the world.
SWAPS Etiquette
SWAPS to be traded are typically kept in a box or baggie. SWAPS that have been pinned on are often what the Scout has collected and are usually off limits unless offered to you.
SWAPS are usually pins, but not always! They may be bracelets, trinkets, or any number of things!
Be polite! The other Scout has put their best effort into making their SWAPS.
Don't pin the SWAP onto the other Scout, allow them to pin it themselves.
Don't refuse an offer to swap, that's considered rude.
If you see a girl with few or no pins to trade, you can offer to simply give her a SWAP!
Never say anything negative about SWAPS you receive.
Make sure to say "thank you" when receiving a SWAP!
Make sure the pin is closed when you SWAP so no one gets poked
If you don't really like a SWAP or have a duplicate, you can SWAP it with someone else.
SWAPS at Jubilation
We recommend bringing at least 60-100 SWAPS per girl, though of course you're welcome to bring more! Girls, Leaders, and Staff will all be happy to swap with you.
You can exchange SWAPS at any time, though particularly good times can include on the bus to off-site activities (for the older girls), between program, when visiting some of the open activities, and during breaks and down time.
